• Work with patients in a long-term care facility — process applications and gather info
• Coordinate services to geriatric patient – process SSA change of addresses
• Act as a liaison between the individual, families, and providers — meet with residents and their families to discuss financial arrangements
• Follow-up with banks, get authorizations signed, make sure admission agreements are signed,
• Set-up referrals for attorney, if needed, gather documents and be familiar with different counties and what forms are needed
• Review Medicaid applications and submit to County for review
• Gather documents for Nursing Home Medicaid approval
• Submit redeterminations

• Must have at least 1 year of experience serving people with long term health conditions
• Strong knowledge of Nursing Home Medicaid application process
• Strong written and verbal communication skills required
• Proficiency in Microsoft Office
